Life! Taking Action on Diabetes
Life! Taking Action on Diabetes is a new type 2 diabetes prevention program. This lifestyle behaviour change program runs over six sessions in groups of between eight and 15 participants. All sessions are facilitated by an experienced health professional, including dietitians, physiotherapists, exercise physiologists and practice nurses.
Participants are given the confidence to make small, achievable changes to improve their lifestyle and learn problem solving skills. Group members are encouraged to reach a weight loss goal through healthy eating and moderate daily exercise. Upon completion of the Life! Course, group members are linked into local support networks (such as exercise groups) to allow them to maintain their improved lifestyle.
Life! Eligibility Criteria
The course is free for most people, including:
- Victorians at high risk of T2DM (AUSDRISK score 12 or more) aged 50 years and over who do not have diabetes
- Aboriginal Victorians aged 18 years and over (AUSDRISK score 12 or more) who do not have diabetes
- Victorians aged 18 years and over who have a history of Gestational Diabetes Mellitus (GDM) or Ischemic Heart Disease (IHD) and have not had a diagnosis of diabetes
- Victorians aged 18 years and over who have had a WorkHealth check and were identified at high risk (AUSDRISK score 12 or more) who do not have diabetes
- There is a co-payment of $50 for those eligible participants who are on high income (as per the Health and Community Care (HACC) guidelines)
